Wills & Trusts
To leave a bequest, first decide on the amount you would like to donate. You may choose to make a gift of a specific dollar amount, a percentage of your estate, or part of what remains after you have remembered family and friends. A charitable bequest can be added to your existing will or trust through a codicil, a simple written amendment that allows you to update your wishes without drafting a new document. Provide your attorney with the following information:
Legal Name: Friends of Metro Animal Services
Address: 3528 Newburg Road, Louisville, KY 40218
Tax ID number: 38-3749218
IRA, Pension, Life Insurance, Donor Advised Fund (DAF), etc.
It is also simple to name Friends of Metro Animal Services as a beneficiary of a retirement fund or other type of asset. Once you decide what percentage you would like to leave, contact your IRA or retirement plan administrator, insurance company, bank or financial institution and request a change-of-beneficiary form. Name Friends of Metro Animal Services as a beneficiary on the form, mail it back to the plan administrator, and keep a copy for your record.
